Help Shape the Future of African American Wellness Services in Contra Costa County
Survey Closes November 20
Black/African American residents of Contra Costa County are invited to participate in an important survey to shape the future of wellness services in the county. Your feedback will directly influence the design, services, and locations of a new African American Holistic Wellness and Resource Hub—a center dedicated to providing culturally relevant and responsive services that meet the unique needs of our community.
The African American community faces disparities in access to health, wellness, and other critical services. This Hub is designed to put the interests and experiences of African American residents at the center of solutions. Your participation ensures that the Hub offers what truly matters to you and your family.
By completing this 15-20 minute survey, you will help shape decisions on services and locations that will best serve the African American community. Your input will directly influence critical choices about the wellness support our community deserves.
As a thank-you for your time, the first 1,000 respondents with valid email addresses will receive a $15 digital Visa gift card!
Take the survey by November 20 to ensure the future African American Holistic Wellness and Resource Hub reflects your needs.
