OUTCALL SERVICES
A Massage Establishment permit is required for the business owner if massage services will be provided within the business.
- Establishment , Independent Contractor, or Outcall Application - $275 new or renewal application fee
- Live Scan fingerprints
- 300 hours of formal massage education
- 200 hours of related training or professional experience (documented)
- Documentation that the applicant has a lawful right to work in the United States
- Proof of massage malpractice insurance
- Proof of membership in a certified massage association
- Permit Fee (when issued)
- Establishment, Owner/Operator, Independent contractor, Outcall - $128
* An establishment applicant who will not personally engage in the practice of massage is exempt from the educational, certification, and association requirements.

MASSAGE THERAPIST
Anyone who provides massage services must obtain a Massage Therapist permit.
- Massage Therapist Application
- $89 new application fee / $70 renewal fee

- $50 new permit / $45 renewal (when issued)
* A Massage Therapist who is employed by an Establishment that carries group malpractice insurance for its employees is exempt from having to obtain a personal malpractice insurance policy.
